Vital Records

Town Vital records pertaining to births, marriages, and deaths are not public information. Individuals may obtain certified copies of vital records Monday through Friday 8 am to 4 pm at the:

If you wish to obtain a certified copy of a Vital Record either in person or through the mail. See Vital Record Applications.

If you are requesting a copy via mail, please provide a copy of your valid Driver' License along with payment payable to the Town of Middletown.

Marriage Licenses

To apply for a marriage license, the applicants must be present by 3 pm with all required documentation. Requirements for the Town of Middletown, to be provided by each applicant, are a certified copy of a Birth Certificate, a valid Driver's License or State Issued ID; if previously married, a Certified copy of your Divorce Decree (Final Judgement), or a Certified Copy of a Death Record, whichever applies.

If your Birth Certificate or Final Judgement is not in English, you must obtain a Full Translation through Pinpoint Translation Services at 401-784-8633. The fee is $24 for the Marriage License, cash, or check (payable to the Town of Middletown).
